With the outsourcing of AA Medical, employee medical records have been archived at the Iron Mountain Data Storage Facility (http://www.ironmountain.com/). For copies of your medical records, see instructions below:
Employee medical records from former AA Medical can be requested by the employee through either a written or emailed request. Requests should include the following information:
·Employee/former employee name
·Employee/former employee ID #
·Method by which record should be returned -- FedEx or email.
·If choosing FedEx, must include an appropriate physical address (no PO boxes)
·Ability to return using email may depend on the size of the file. Note: emailed files will not be encrypted and there is some level of risk that the information in the email could be read by a third party.
