  • Enrollment for your 2022 benefits has ended, but you may still have more to do

    Annual Enrollment is over, and you can check “Enroll in 2022 benefits” off your to-do list. There may still be a couple more to-dos before you can use your benefits in the new year.

    To help you out, here’s a checklist to make sure you’re all set before 2022.

    Review your benefits elections

    After you enrolled through the Benefits Service Center, you should have received a Benefits Enrollment Summary sent to your preferred email address. Be sure to review your summary to make sure everything is correct. If you see any issues, call the Benefits Service Center at 888-860-6178, Monday through Friday, 8 a.m. to 5 p.m. CT.

     Watch for ID cards to arrive

    You can expect new ID cards to arrive in the mail in December 2021. Here’s what to watch for:

      • This year, all team members enrolled in an American medical option will receive a new ID card and a CVS Caremark card.
      • All team members enrolled in an American medical option (except those in the DFW ConnectedCare plan) will also receive a member ID card for SurgeryPlus. If you choose, you may take advantage of this voluntary program that offers support if you will be having surgery.
      • If you enrolled in vision coverage for the first time, you’ll receive an EyeMed ID card.

     Watch for your Smart-Choice debit card to arrive

    If you don’t currently have a Smart-Choice Accounts debit card, you may receive a debit card in the mail, depending on your 2022 elections.

      • If you elected a flexible spending account for 2022, you will receive a debit card in late December 2021.
      • If you elected the Core medical option and set a goal amount for your health savings account (HSA) for 2022, you’ll receive your debit card in late December 2021. You can set or change your HSA goal amount at any time during the year online through the Benefits Service Center.
      • If you elected the Plus plan, once you receive your preventive care exam, your health reimbursement arrangement will automatically be opened with your $500 in credits and your debit card will be sent to you.

    If you didn’t have a Smart-Choice Accounts debit card in 2021 but will be opening a health spending or savings account (e.g., health care flexible spending account, health savings account, etc.) or receive health reimbursement arrangement credits for the first time in 2022, you’ll receive a debit card when your account is activated.

     Provide proof of good health, if needed

    Most of your coverage will go into effect as of Jan. 1, 2022. However, some benefits, such as voluntary life and disability insurance, will only go into effect next year after your proof of good health is approved. If you didn’t provide proof of good health during the enrollment process, you will receive either a form in the mail or an email with a link to the forms that need to be completed. Please submit those forms by the deadline provided.

     Verify your dependents

    For any newly enrolled dependents — a spouse or domestic partner, any children under age 26 or another dependent — you’ll need to provide documentation to verify their relation to you. Look for a notification sent to your preferred email address with your individual due date to submit documentation, such as a birth or marriage certificate.
